Introducción: El Síndrome de Burnout (SB) surge en el contexto de condiciones laborales precarias, y se manifiesta a través de un conjunto de signos y síntomas que afectan la psicología del trabajador debido a la exposición a factores estresantes en el ambiente laboral.Objetivo: Analizar la prevalencia y los factores asociados al Síndrome de Burnout (SB) en enfermeras de cuidados intensivos. Método: Estudio transversal y analítico, realizado con 94 enfermeros asistenciales en unidades de cuidados intensivos de un hospital público en el estado de Bahía. Se utilizó el Inventario de Burnout de Maslach (MBI) y un cuestionario que contenía aspectos sociodemográficos, culturales y ocupacionales. Resultados: El 62,8% mostró alto agotamiento emocional; 64,9% alta despersonalización y 77,7% alta reducción de la realización profesional. Los factores asociados al SB fueron: edad hasta 38 años (RP: 2,38; IC 95%: 1,44-3,94), no tener pareja (RP: 1,97; IC 95%: 1,17-3,32), insatisfacción laboral (RP: 1,78; IC 95%: 1,15 -2,75), tener dolor de espalda (RP: 3,33; IC 95%: 1,72 -6,42), ansiedad (RP: 2,33; IC 95%: 1,22-4,46) y patrón de sueño hasta 5 horas (RP: 1,67; IC 95%: 1,08-2,59). Se encontró que tener hijos es un índice protector (RP: 0,55; IC 95%: 0,33-0,90). Verificou-se que ter filhos é um índice protetor (RP: 0,55; IC 95%: 0,33-0,90). Conclusão: Existe um percentual considerado elevado de enfermeiros com diagnóstico sugestivo da síndrome (43,6%), assim como a existência de fatores associados. É imprescindível que a instituição ofereça um ambiente de trabalho sadio, no qual, a prevenção é a melhor maneira para que os profissionais não se tornem alvos da síndrome. (AU)


Introduction: Burnout Syndrome (BS) emerges in the context of precarious working conditions, and manifests itself through a set of signs and symptoms that affect the worker's psychology due to exposure to stressors in the work environment. Objective: to analyze the prevalence and factors associated with Burnout Syndrome (BS) in intensive care nurses. Method: cross-sectional and analytical study, carried out with 94 nurses in intensive care units of a public hospital in the state of Bahia. The Maslach Burnout Inventory (MBI) and a questionnaire containing sociodemographic, cultural and occupational issues were used. Results: 62.8% showed high emotional exhaustion; 64.9% high depersonalization and 77.7% high reduction in professional achievement. The factors associated with BS were: age up to 38 years (PR: 2.38; 95% CI: 1.44-3.94), not having a partner (PR: 1.97; 95% CI: 1.17-3.32), job dissatisfaction (PR: 1.78; 95% CI: 1.15 -2.75), having back pain (PR: 3.33; 95% CI: 1.72 -6.42), anxiety (PR: 2.33; 95% CI: 1.22-4.46) and sleep pattern up to 5 hours (PR: 1.67; 95% CI: 1.08-2.59). It was found that having children is a protective index (PR: 0.55; 95% CI: 0.33-0.90). Conclusion: there is a considered high percentage of nurses with a diagnosis suggestive of the syndrome (43.6%), as well as the existence of associated factors. It is essential that the institution offers a healthy work environment, in which prevention is the best way for these professionals not to become targets of the syndrome. (AU)

BACKGROUND: Global access to acute stroke treatment is variable worldwide, with notable gaps in low and middle-income countries (LMIC), especially in rural areas. Ensuring a standardized method for pinpointing the existing regional coverage and proposing potential sites for new stroke centers is essential to change this scenario. AIMS: To create and apply computational strategies (CSs) to determine optimal locations for new acute stroke centers (ASCs), with a pilot application in nine Latin American regions/countries. METHODS: Hospitals treating acute ischemic stroke (AIS) with intravenous thrombolysis (IVT) and meeting the minimum infrastructure requirements per structured protocols were categorized as ASCs. Hospitals with emergency departments, noncontrast computed tomography (NCCT) scanners, and 24/7 laboratories were identified as potential acute stroke centers (PASCs). Hospital geolocation data were collected and mapped using the OpenStreetMap data set. A 45-min drive radius was considered the ideal coverage area for each hospital based on the drive speeds from the OpenRouteService database. Population data, including demographic density, were obtained from the Kontur Population data sets. The proposed CS assessed the population covered by ASCs and proposed new ASCs or artificial points (APs) settled in densely populated areas to achieve a target population coverage (TPC) of 95%. RESULTS: The observed coverage in the region presented significant disparities, ranging from 0% in the Bahamas to 73.92% in Trinidad and Tobago. No country/region reached the 95% TPC using only its current ASCs or PASCs, leading to the proposal of APs. For example, in Rio Grande do Sul, Brazil, the introduction of 132 new centers was suggested. Furthermore, it was observed that most ASCs were in major urban hubs or university hospitals, leaving rural areas largely underserved. CONCLUSIONS: The MAPSTROKE project has the potential to provide a systematic approach to identify areas with limited access to stroke centers and propose solutions for increasing access to AIS treatment. BACKGROUND: Randomized trials have recently evaluated the non-inferiority of direct thrombectomy versus intravenous thrombolysis (IVT) followed by endovascular therapy in anterior circulation large vessel occlusion (LVO) stroke in patients eligible for IVT within 4.5 h from stroke onset with controversial results. We aimed to assess the effect of IVT on the clinical outcome of mechanical thrombectomy (MT) in the RESILIENT trial. METHODS: RESILIENT was a randomized, prospective, multicenter, controlled trial assessing the safety and efficacy of thrombectomy versus medical treatment alone. A total of 221 patients were enrolled. The trial showed a substantial benefit of MT when added to medical management. All eligible patients received intravenous tPA within the 4.5-h-window. Ordinal logistic and binary regression analyses using intravenous tPA as an interaction term were performed with adjustments for potential confounders, including age, baseline NIHSS score, occlusion site, and ASPECTS. A p-value <0.05 was considered statistically significant. RESULTS: Among 221 randomized patients (median NIHSS, 18 IQR [14-21]), 155 (70%) were treated with IVT. There was no difference in the mRS ordinal shift and frequency of functional independence between patients who received or not IV tPA; the odds ratio for the ordinal mRS shift was 2.63 [1.48-4.69] for the IVT group and 1.54 [0.63-3.74] for the no IVT group, with a p-value of 0.42. IVT also did not affect the frequency of good recanalization (TICI 2b or higher) and hemorrhagic transformation. CONCLUSIONS: The large effect size of MT on LVO outcomes was not significantly affected by IVT. TRIAL REGISTRATION: RESILIENT ClinicalTrials.gov number, NCT02216643.

This study evaluated the effect of prepubertal arsenic exposure in the liver and kidney of pubescent rats and their reversibility 30 days after arsenic withdrawal. Male pups of Wistar rats (21 days old) were divided into two groups (n = 20/group): control animals received filtered water, and exposed rats received 10 mg L-1 arsenic from postnatal day (PND) 21 to PND 51. The liver and kidney of 52 days old rats (n = 10/group) were examined to investigate the effects of arsenic on micromineral content, antioxidant enzyme activity, histology, and biochemistry parameters. The other animals were kept alive under free arsenic conditions until 82 days old and further analyzed by the same parameters. Our results revealed that 52-day-old rats increased arsenic content in their liver and arsenic and manganese in their kidney. In those animals, glycogen and zinc content and catalase activity were reduced in the liver, and the selenium content decreased in the kidney. Thirty days later, arsenic reduced the manganese and iron content and SOD and CAT activity in the liver of 82-day-old rats previously exposed to arsenic, while glycogen and selenium content decreased in their kidney. In contrast, PND 82 rats exhibited higher retention of copper in the liver, an increase in iron and copper content, and CAT and GST activity in the kidney. Significant histological alterations of liver and kidney tissues were not observed in rats of both ages. We conclude that arsenic-induced toxicity could alter differently the oxidative status and balance of trace elements in pubertal and adult rats, demonstrating that the metalloid can cause effects in adulthood.

Abstract Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.


Resumo Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

INTRODUCTION: The growing cost of stroke care has created the need for outcome-oriented and cost-saving payment models. Identifying imbalances in the current reimbursement model is an essential step toward designing impactful value-based reimbursement strategies. This study describes the variation in reimbursement fees for ischemic stroke management across the USA. METHODS: This Medicare Fee-For-Service claims study examines USA beneficiaries who suffered an ischemic stroke from 2021Q1 to 2022Q2 identified using the Medicare-Severity Diagnosis-Related Groups (MS-DRGs). Demographic national and regional US data were extracted from the Census Bureau. The MS-DRG codes were grouped into four categories according to treatment modality and clinical complexity. Our primary outcome of interest was payments made across individual USA and US geographic regions, assessed by computing the mean incremental payment in cases of comparable complexity. Differences between states for each MS-DRG were statistically evaluated using a linear regression model of the logarithmic transformed payments. RESULTS: 227,273 ischemic stroke cases were included in our analysis. Significant variations were observed among all DRGs defined by medical complexity, treatment modality, and states (p < 0.001). Differences in mean payment per case with the same MS-DRG vary by as high as 500% among individual states. Although higher payment rates were observed in MS-DRG codes with major comorbidities or complexity (MCC), the variation was more expressive for codes without MCC. It was not possible to identify a standard mean incremental fee at a state level. At a regional level, the Northeast registered the highest fees, followed by the West, Midwest, and South, which correlate with poverty rates and median household income in the regions. CONCLUSIONS: The payment variability observed across USA suggests that the current reimbursement system needs to be aligned with stroke treatment costs. Future studies may go one step further to evaluate accurate stroke management costs to guide policymakers in introducing health policies that promote better care for stroke patients.

Pulmonary arterial hypertension (PAH) is characterized by elevated arterial pressure and vascular resistance. PAH may cause alterations in the microcirculation of several organs, including the kidney, liver, brain, and testes. However, it remains unclear whether monocrotaline-induced PAH exerts detrimental effects on animal testes. Thus, we analyzed the impact of PAH on testicular morphology and function. Additionally, we investigated the effect of resistance exercise training (RT) on testicular parameters in PAH rats. Eight healthy Wistar rats and eight PAH rats were subjected to RT training for 30 days; the other PAH and healthy rats (n = 8/group) did not exercise. PAH rats had lower reproductive organ weight, serum testosterone levels, testicular glucose, and nitric oxide (NO) levels, Leydig cell parameters, tubular morphometry, germ cell counts, and daily sperm production than healthy animals did. The practice of RT attenuated the negative impact of PAH on the relative weights of the testes and epididymides, Leydig cell number, nuclear volume, testicular NO levels, and seminiferous epithelium architecture. Moreover, RT positively influenced testosterone levels in PAH animals. We conclude that PAH exerts deleterious effects on testicular histology and function. However, RT can be beneficial to the PAH-affected testicular parameters.

RESUMO

Local and regional cooperation may strengthen efforts to reduce stroke burden in low-resource settings. New generations full of energy, honoring the past achievements of mentors and current stroke leaders, have the potential to stimulate stroke research, and prevention and implement the available evidence-based treatments. This article presents how a local initiative focused on young stroke professionals may promote comprehensive stroke care in the region. We will present the creation of ALATAC, its main purpose and objectives, the structure organization, the committees, ongoing activities, the potential results to be achieved, and how to become a member of this group.

Background: The global COVID-19 pandemic has had a devastating effect on global health, resulting in a strain on healthcare services worldwide. The faster a patient with acute ischemic stroke (AIS) receives reperfusion treatment, the greater the odds of a good functional outcome. To maintain the time-dependent processes in acute stroke care, strategies to reorganize infrastructure and optimize human and medical resources were needed. Methods: Data from AIS patients who received thrombolytic therapy were prospectively assessed in the emergency department (ED) of Hospital de Clínicas de Porto Alegre from 2019 to 2021. Treatment times for each stage were measured, and the reasons for a delay in receiving thrombolytic therapy were evaluated. Results: A total of 256 patients received thrombolytic therapy during this period. Patients who arrived by the emergency medical service (EMS) had a lower median door-to-needle time (DNT). In the multivariable analysis, the independent predictors of DNT >60 min were previous atrial fibrillation (OR 7) and receiving thrombolysis in the ED (OR 9). The majority of patients had more than one reason for treatment delay. The main reasons were as follows: delay in starting the CT scan, delay in the decision-making process after the CT scan, and delay in reducing blood pressure. Several actions were implemented during the study period. The most important factor that contributed to a decrease in DNT was starting the bolus and continuous infusion of tPA on the CT scan table (decreased the median DNT from 74 to 52, DNT ≤ 60 min in 67% of patients treated at radiology service vs. 24% of patients treated in the ED). The DNT decreased from 78 min to 66 min in 2020 and 57 min in 2021 (p = 0.01). Conclusion: Acute stroke care continued to be a priority despite the COVID-19 pandemic. The implementation of a thrombolytic bolus and the start of continuous infusion on the CT scan table was the main factor that contributed to the reduction of DNT. Continuous monitoring of service times is essential for improving the quality of the stroke center and achieving better functional outcomes for patients.

ETHNOPHARMACOLOGICAL RELEVANCE: Pfaffia glomerata (Spreng.) Pedersen has traditionally been used as a tonic and a stimulant by the Brazilian population. It shows higher biomass accumulation and production of secondary compounds, such as the phytosterol 20-hydroxyecdysone. AIMS: The present study aimed to evaluate the effects of the hydroalcoholic extract of the root of tetraploid P. glomerata (BGEt) on testicular parenchyma, and its implications on fertility. MATERIAL AND METHODS: Adult Swiss mice were divided as: control (water) and sildenafil citrate (7 mg/kg), BGEt at 100, 200, and 400 mg/kg, and BGEtD 200 mg/kg (treated with BGE every three days). Males (n = 4/group) were mated with normal untreated adult females to assess fertility rates, while other animals (n = 6/group) were euthanized for testis, epididymis, and oxidative stress analyses. RESULTS: Increase in tubule diameter and epithelium height in the discontinuous group, in addition to an increase in the proportion of tubules with moderate pathologies was observed. The pre-implantation loss was lower in all treated groups. The post-implantation loss was significantly increased in all treated groups, except for the lowest BGEt dose. BGEt intake caused a decrease in daily sperm production, along with the number and quality of sperm in the epididymis. Changes were observed in protein carbonylation and hydrogen peroxide and nitric oxide levels, characterizing oxidative stress. CONCLUSIONS: The hydroalcoholic extract of P. glomerata tetraploid altered sperm and testicular parameters, compromising embryonic development after implantation.

Arsenic is an environmental toxicant known to be a carcinogen and endocrine disruptor. Maternal exposure to arsenic has been associated with fetus malformation and reproductive disorders in male offspring. However, it is unclear the extent to which those effects remain during postnatal development and adulthood. Therefore, this study aimed to investigate the long-term effects of prenatal arsenic exposure on reproductive parameters of male offspring at peripubertal and adult periods. Pregnant female Wistar rats were exposed to 0 or 10 mg/L sodium arsenite in drinking water from gestational day 1 (GD 1) until GD 21 and male pups were analyzed at postnatal day 44 (PND 44) and PND 70. We observed that some reproductive parameters were affected differently by arsenic exposure at each age evaluated. The body and reproductive organs weights, as well as testicular and epididymal morphology were strongly affected in peripubertal animals and recovered at adult period. On the other hand, the antioxidant genes expression (SOD1, SOD2, CAT and GSTK1) and the endogenous antioxidant system were affected in the testes and epididymides from both peripubertal and adult rats. Finally, an impairment in daily sperm production and in sperm parameters was observed in adult animals. Taken together, our findings show that prenatal arsenic exposure affected reproductive parameters of peripubertal and adult male rats mainly due to oxidative stress. Collectively, those alterations may be affecting fertility potential of adult animals.

Due to environmental contamination, the environment constantly receives pollutants from various anthropic actions. These pollutants put ecological health at risk due to contamination and accumulation in living organisms, including wild animals and humans. Exposure can cause physiological, morphological, and behavioral changes in living beings. In this context, laboratory studies have frequently investigated how environmental contaminants affect the male reproductive system and gametes. However, few studies have examined how these contaminants affect male reproduction in naturally exposed animals. To better understand this topic, we conducted a systematic review of the effects of exposing male vertebrate animals to polluted environments on their reproductive functions. After an extensive search using the PubMed/MEDLINE, Scopus, and Web of Science databases, 39 studies met our inclusion criteria and were eligible for this review. This study showed that reproductive damages were frequent in fishes, amphibians, reptiles, birds, and mammals exposed to contaminated environments. Wild animals are exposed mainly to endocrine-disrupting compounds (EDCs), toxic metals, and radiation. Exposure to pollutants causes a reduction in androgen levels, impaired spermatogenesis, morphological damage to reproductive organs, and decreased sperm quality, leading to reduced fertility and population decline. Although several species have been studied, the number of studies is limited for some groups of vertebrates. Wildlife has proven valuable to our understanding of the potential effects of environmental contaminants on human and ecosystem health. Thus, some recommendations for future investigations are provided. This review also creates a baseline for the understanding state of the art in reproductive toxicology studies.

Resumo Objetivo Mensurar a cultura de segurança do paciente na perspectiva dos enfermeiros de Unidades de Terapia Intensiva. Métodos Estudo descritivo transversal desenvolvido com 65 enfermeiros assistenciais das Unidades de Terapia Intensiva de um hospital público terciário por meio do preenchimento do questionário Hospital Survey on Patient Safety Culture, utilizando a estatística descritiva simples para análise dos dados, seguindo as orientações da Agency for Healthcare Research and Quality's. Resultados Do total, 50% dos enfermeiros não realizaram nenhuma notificação de evento adverso nos últimos 12 meses; 50,8% dos profissionais atribuíram nota Regular à Segurança do Paciente na sua área/unidade de trabalho no hospital; nenhuma das Dimensões apresentou taxa de respostas positivas >75%, apenas itens isolados como o C2 ("Os profissionais têm liberdade para dizer ao ver algo que pode afetar negativamente o cuidado do paciente"), com 84,6%. As Dimensões 6 (Respostas não punitivas aos erros), 9 (Trabalho em equipe entre as unidades hospitalares) e 11 (Percepções gerais sobre segurança) apresentaram todos os itens com percentual médio abaixo de (50%); a Dimensão 6 ("Respostas não punitivas aos erros") foi a mais frágil (22,5%). Conclusão Este estudo evidenciou baixa taxa de notificação de eventos adversos e percepção dos enfermeiros de uma cultura punitiva por parte de seus superiores; evidenciaram-se lacunas na cultura de segurança que precisam ser reavaliadas para buscar estratégias de melhoria e fortalecimento do cuidado, tornando a assistência, cada vez mais, qualificada e segura.


Resumen Objetivo Medir la cultura de seguridad del paciente bajo la perspectiva de los enfermeros de Unidades de Cuidados Intensivos. Métodos Estudio descriptivo transversal llevado a cabo con 65 enfermeros asistenciales de las Unidades de Cuidados Intensivos de un hospital público terciario por medio del cuestionario Hospital Survey on Patient Safety Culture ; se utilizó la estadística descriptiva simple para el análisis de los datos; y se siguieron las instrucciones de la Agency for Healthcare Research and Quality. Resultados Del total de los enfermeros, el 50 % no realizó ninguna notificación de evento adverso en los últimos 12 meses; el 50,8 % de los profesionales calificó como Regular la seguridad del paciente en su área/unidad de trabajo del hospital; ninguna de las dimensiones presentó índice de respuestas positivas >75 %, solo en ítems aislados como el C2 ("Los profesionales tienen libertad para decir si ven algo que puede afectar negativamente el cuidado del paciente"), con un 84,6 %. La dimensión 6 (Respuesta no punitiva a los errores), la dimensión 9 (Trabajo en equipo entre unidades) y la 11 (Percepción global de seguridad) presentaron un porcentaje promedio inferior a (50 %) en todos los ítems; la dimensión 6 (Respuesta no punitiva a los errores) fue la más frágil (22,5 %). Conclusión Este estudio evidenció un bajo índice de notificación de eventos adversos y la percepción de los enfermeros de una cultura punitiva por parte de sus superiores. Se observaron vacíos en la cultura de seguridad que deberían ser reevaluados para buscar estrategias de mejora y fortalecimiento del cuidado, para que la atención sea cada vez más calificada y segura.


Abstract Objective To measure patient safety culture from Intensive Care Unit nurses' perspective. Methods This is a cross-sectional descriptive study carried out with 65 nurses from Intensive Care Units of a tertiary public hospital, by completing the Hospital Survey on Patient Safety Culture questionnaire, using simple descriptive statistics for data analysis and following the Agency for Healthcare Research and Quality's guidelines. Results Of the total, 50% of nurses did not report any adverse events in the last 12 months; 50.8% of professionals assigned a fair score to patient safety in their work area/unit at the hospital; none of the dimensions had a rate of positive responses >75%, only isolated items such as C2 (Professionals are free to say when they see something that may negatively affect patient care), with 84.6%. Dimensions 6 (Nonpunitive responses to errors), 9 (Teamwork across hospital units) and 11 (Overall perceptions of safety) showed all items with an average percentage below (50%), and Dimension 6 (Nonpunitive responses to errors) was the weakest (22.5%). Conclusion This study showed a low rate of adverse event reporting and nurses' perception of a punitive culture on the part of their superiors. Gaps in the safety culture were evidenced, which need to be reassessed in order to seek strategies to improve and strengthen care, making care increasingly qualified and safe.

The field triage of stroke patients should consider a careful balance between the best destination for the timely delivery of intravenous and/or endovascular reperfusion therapies. The Field Assessment Stroke Triage for Emergency Destination (FAST-ED) scale has been shown to have an accuracy comparable to that of the National Institutes of Health Stroke Scale (NIHSS). However, it has not been tested in the field. OBJECTIVE: To evaluate the accuracy of the FAST-ED scale in the detection of AIS due to large vessel occlusion (LVO) in the prehospital setting. METHODS: A cross-sectional study of consecutive prospective data collected from February 2017 to May 2019 in the city of Porto Alegre, state of Rio Grande do Sul, Southern Brazil, correlating the prehospital FAST-ED scale scores with the hospital diagnosis of LVO. Area under the curve (AUC), sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated. RESULTS: In total, 74 patients were included in the analysis. As compared with the diagnosis of LVO upon hospital discharge, the prehospital FAST-ED scale applied by paramedics had a sensitivity of 80%, a specificity of 47.7%, a PPV of 51.1%, an NPV of 77.8%, and an AUC of 0.68 (95% confidence interval [95%CI]: 0.55-0.80). Among the patients with a final diagnosis of AIS, the accuracy was higher, with an AUC of 0.75 (95%CI: 0.60-0.89), a sensitivity of 80%, a specificity of 60%, a PPV of 80%, and an NPV of 60%. CONCLUSIONS: In the present study, the FAST-ED scale, which was applied by paramedics in the field, demonstrated moderate accuracy but high sensitivity and NPV, which are essential attributes for a triage scale. While larger studies are still needed, these findings further support the use of the FAST-ED in stroke triage.

MéTODOS: Estudo transversal de dados prospectivos consecutivos, coletados de fevereiro de 2017 a maio de 2019, em Porto Alegre, Rio Grande do Sul, Brasil, em que se correlacionam a pontuação pré-hospitalar na escala FAST-ED e o diagnóstico hospitalar de OGV. A área sob a curva (ASC), a sensibilidade, a especificidade, o valor preditivo positivo (VPP), e o valor preditivo negativo (VPN) foram calculados. RESULTADOS: Ao todo, 74 pacientes foram incluídos na análise. Comparada ao diagnóstico de OGV na alta hospitalar, a escala FAST-ED aplicada em campo por profissionais do pré-hospitalar teve sensibilidade de 80%, especificidade de 47,7%, VPP de 51,1%, VPN de 77,8%, e ASC de 0,68 (intervalo de confiança de 95% [IC95%]: 0,55­0,80). Entre pacientes com diagnóstico final de AVCI, a precisão foi mais alta, com ASC de 0,75 (IC95%: 0,60­0,89), sensibilidade de 80%, especificidade de 60%, VPP de 80%, e VPN de 60%. CONCLUSõES: Neste estudo, a escala FAST-ED, aplicada por profissionais do pré-hospitalar em campo, demonstrou precisão moderada, com alta sensibilidade e VPN, atributos essenciais para uma escala de triagem. Embora estudos com amostras maiores ainda sejam necessários, estes achados apoiam o uso da FAST-ED na triagem de AVCI.

The unsustainable increases in healthcare expenses and waste have motivated the migration of reimbursement strategies from volume to value. Value-based healthcare requires detailed comprehension of cost information at the patient level. This study introduces a clinical risk- and outcome-adjusted cost estimate model for stroke care sustained on time-driven activity-based costing (TDABC). In a cohort and multicentre study, a TDABC tool was developed to evaluate the costs per stroke patient, allowing us to identify and describe differences in cost by clinical risk at hospital arrival, treatment strategies and modified Rankin Score (mRS) at discharge. The clinical risk was confirmed by multivariate analysis and considered patients' National Institute for Health Stroke Scale and age. Descriptive cost analyses were conducted, followed by univariate and multivariate models to evaluate the risk levels, therapies and mRS stratification effect in costs. Then, the risk-adjusted cost estimate model for ischaemic stroke treatment was introduced. All the hospitals collected routine prospective data from consecutive patients admitted with ischaemic stroke diagnosis confirmed. A total of 822 patients were included. The median cost was I$2210 (interquartile range: I$1163-4504). Fifty percent of the patients registered a favourable outcome mRS (0-2), costing less at all risk levels, while patients with the worst mRS (5-6) registered higher costs. Those undergoing mechanical thrombectomy had an incremental cost for all three risk levels, but this difference was lower for high-risk patients. Estimated costs were compared to observed costs per risk group, and there were no significant differences in most groups, validating the risk and outcome-adjusted cost estimate model. By introducing a risk-adjusted cost estimate model, this study elucidates how healthcare delivery systems can generate local cost information to support value-based reimbursement strategies employing the data collection instruments and analysis developed in this study.

Introduction: Acute stroke interventions, such as stroke units and reperfusion therapy, have the potential to improve outcomes. However, there are many disparities in patient characteristics and access to the best stroke care. Thus, we aim to compare patient-reported outcome measures (PROMs) after stroke in two stroke centers representing the public and private healthcare systems in Brazil. Methods: PROMs through the International Consortium for Health Outcomes Measures (ICHOM) were assessed at 90 days after the stroke to compare two Brazilian hospitals in southern Brazil: a public university and a private stroke center, both with stroke protocols and stroke units. Results: When compared with the private setting (n = 165), patients from the public hospital (n = 175) were younger, had poorer control of risk factors, had more frequent previous strokes, and arrived with more severe strokes. Both hospitals had a similar percentage of IV thrombolysis treatment. Only 5 patients received mechanical thrombectomy (MT), all in the private hospital. Public hospital patients presented significantly worse outcomes at 3 months, including worse quality of life and functional dependence (60 vs. 48%, p = 0.03). Poor outcome, as measured by the mRS score, was significantly associated with older age, higher NIHSS score, and the presence of heart failure. However, the public practice was a strong predictor of any self-reported disability. Conclusion: Patients assisted at a good quality public stroke center with the same protocol used in the private hospital presented worse disability as measured by mRS and patient-reported outcome measures, with greater inability to communicate, dress, toilet, feed, and walk.
